My 59 beetle, once owned by my grandmother has one piece trafficator lenses screwed over the semaphore slot. Does anyone know if this is how some were delivered from the factory in Aug 59, or did it once have semaphores that were removed some time ago?

No, all 59s delivered to Europe, SA, Australia etc had semaphores. Any variation on that was an after-market conversion like these on a Kombi:
